Family Nurse Practitioner (FNP)

Position Summary


The Family Nurse Practitioner (FNP) will collaborate with a dedicated clinical care team to provide evidence-based primary and family healthcare services. This role is responsible for assessing, diagnosing, treating, and managing acute and chronic health conditions while promoting preventive care and patient wellness. The FNP will contribute to improving patient outcomes through high-quality, patient-centered care.

Required Qualifications

  • Master's Degree from an accredited Family Nurse Practitioner program.
  • Current National Board Certification as a Family Nurse Practitioner.
  • Active Nurse Practitioner license in good standing within the state of practice.
  • Active RN and APRN licenses with no current or prior disciplinary actions.
  • Current BLS certification through the American Heart Association (AHA) or American Red Cross (ARC).
  • Minimum of 1 year of recent Nurse Practitioner experience in one or more of the following settings:
    • Family Medicine
    • Primary Care
    • Urgent Care
  • Strong knowledge of chronic disease management and quality care measures.
  • Excellent verbal, written, and electronic communication skills.
  • Strong organizational, time-management, and multitasking abilities.
  • Ability to work independently and collaboratively within a multidisciplinary clinical team.
  • Proficiency with Electronic Medical Records (EMR/EHR) systems.
  • Willingness to obtain additional state licensure if required.

Key Responsibilities

  • Provide comprehensive primary and family healthcare services.
  • Perform patient assessments, diagnoses, treatment planning, and follow-up care.
  • Conduct routine physical examinations and diagnostic evaluations.
  • Manage acute and chronic medical conditions.
  • Educate patients on preventive care, healthy lifestyle choices, and wellness strategies.
  • Coordinate care with physicians, specialists, and other healthcare professionals.
  • Support chronic disease management and population health initiatives.
  • Complete clinic administrative duties, including:
    • Opening and closing procedures
    • Inventory management
    • Insurance verification
    • Patient follow-up
    • Maintaining a clean, organized, and safe clinical environment

Clinic Operating Hours

  • Monday – Friday: 8:30 AM – 7:30 PM
  • Saturday: 9:00 AM – 5:30 PM
  • Sunday: 9:00 AM – 4:30 PM


Weekend and holiday rotations may be required based on business needs.
